Advancing palliative care.

Improving quality of life.

The OutCare Foundation is an independent, registered charity dedicated to raising funds for community-based palliative care in Eastern Ontario. We support the delivery of palliative services into home to ease the burden on families and the healthcare system.

Here Because
We Care

We’re driven to fund palliative care initiatives because we’ve seen first-hand how quality care improves the lives of families, individuals, and communities.

We are here to make sure people living with serious illness, at every stage and any age, have access to specialized, patient-centred care that treats the symptoms and the stress of living with a serious illness. Why? Because all of our families deserve nothing less.

Why Donate?

The OutCare Foundation supports palliative care programs and needs not funded by the government. We are in regular contact with our hospice network, on the front line,  to identify priority needs, funding gaps and opportunities for palliative care across the Champlain Local Health Integration Network within Eastern Ontario.

But we need your help. Your donation can make all the difference in the lives of palliative patients, their circle of care, and our community. There are many ways to give – from one-time donations, monthly giving, legacy and more – it all helps to ensure palliative care is here when you need it.
