The Hospice Association of Ontario has an organization search tool on their website to help you find organizations in your community. The Champlain Hospice Palliative Care Program coordinates the delivery of palliative care in the Champlain region.

Support for patients who wish to remain in their homes

Residential hospices

  • Carefor Cornwall Hospice is a 10-bed facility that provides end-of-life care in a home-like setting to the residents of the city of Cornwall, Stormont, Dundas, and Glengarry, as well as Prescott and Russell Counties
  • Hospice Care Ottawa (Central West Hospice and May Court Hospice) is an amalgamation of Friends of Hospice Ottawa and the Hospice at May Court, is a community based charitable organization. They offer services for clients and their loved ones to experience palliative and end-of-life care in a supportive and peaceful setting, surrounded by caring that reflects as closely as possible a comfortable home environment.
  • Hospice Renfrew is a comfortable, home-like palliative care residence with 6 private bedrooms located in the heart of Renfrew, but serving individuals and families throughout Renfrew County and the surrounding area
  • Diane Morrison Hospice is a 14-bed facility that provides around-the-clock care, offering needy individuals the opportunity to spend the final chapter of their lives in a supportive environment surrounded by people who care. The Mission serves Ottawa’s homeless.
  • Roger’s House provides a “home away from home” for children with life-limiting illnesses and their families.
  • Madawaska Valley Hospice will bring a two-bed hospice to Barry’s Bay to provide support close to home for individuals in the Barry’s Bay and surrounding communities.
